When going on a Scouting related activity, health information is required. PDF was created using for fields, so it's a little easier to fill out. Too bad every year seems to come up with format changes making it difficult to reuse for future outings.

Brother Randall L. Ridd spoke during the evening fireside and our area authority, Elder Basset, was also in attendance. It was a fun event for our young men.
